Use this form to cancel your active Lease-to-Own subscription.

Please note: The Lease-to-Own subscription has a mandatory minimum term of 6 months. If you submit a cancellation request before this period ends, your cancellation will take effect at the end of your 6th month.

Important Hardware Return: As this is a hardware lease, you are contractually obligated to return the niostem device upon the effective date of your termination. Our system will immediately confirm receipt of your request via email. Our support team will then follow up within 1-2 business days with your exact termination date and return instructions. Failure to return the device may result in continued charges as outlined in our Terms of Service.
